Medically prescribed heroin is more cost-effective than methadone for treating long-term street heroin users, according to a new study by researchers at Providence Health Care and the University of British Columbia.

The study, published today in the Canadian Medical Association Journal (CMAJ), attributed most of the economic benefits to the fact that recipients of medically prescribed heroin (diacetylmorphine) stayed in treatment longer and spent less time in relapse than those receiving methadone. Both results are associated with reduced criminal activity and lower health care costs.

Additionally, an individual in the diacetylmorphine group was more likely to live longer than someone receiving methadone maintenance therapy.

The findings were drawn from the North American Opiate Medication Initiative (NAOMI), a trial of medically prescribed heroin that took place in Vancouver from 2005 to 2008, as well as administrative drug data from British Columbia. NAOMI was North America's first-ever clinical trial of prescribed heroin.

The researchers ? led by Dr. Aslam Anis, director of the Centre for Health Evaluation and Outcome Sciences (CH?OS) at Providence Health Care and professor in UBC's School of Population and Public Health ? used a cost-effectiveness analysis to compare treatments over a one-, five- and 10-year period, as well as a lifetime horizon.

Those in the methadone therapy group generated an average lifetime societal cost of $1.14 million per person, while those in the diacetylmorphine group generated a lesser cost of $1.09 million. The study considered treatment expenditures, estimated costs for drug therapies and costs borne by the justice system.

"NAOMI demonstrated that heroin-assisted therapy is a more effective treatment option than MMT, but now, thanks to this study, we can also confidently say that there are significant economic benefits of using this medication," Dr. Anis says.

"The question I get most about heroin-assisted therapy is whether we can afford the increased direct costs of the treatment," says co-author Dr. Martin Schechter, a CH?OS scientist and professor at UBC's School of Population and Public Health. "What this study shows is that the more appropriate question is whether we can afford not to."

The NAOMI study, which was also led by researchers from PHC, CH?OS and UBC, was a randomized trial aimed at testing whether medically prescribed heroin was more effective than methadone therapy for individuals who were not currently benefiting from conventional treatment.

The results, published in the New England Journal of Medicine, showed that patients treated with the prescribed heroin were more likely to stay in treatment or quit heroin altogether and more likely to reduce their use of illegal drugs and other illegal activities than patients treated with oral methadone.

Business Success Skills Delegation and Teambuilding Share the Load

Here is a really important success tip for business leaders and it is all about delegating. Since delegation is a key to growing your business AND delegation is a skill we are never taught in school - just what is an effective way to delegate in your business?

And here is the story that prompts this article.

I was working with a client this week whose company just put on their first expo, a weekend event. There were hundreds of other companies there. It was a tremendous success, huge amounts of positive feedback and new business. I was congratulating her profusely when she said this;

"And I had to work four 80-hour weeks to get it to happen." I could feel the exhaustion in her voice. And it is not like she did not have a staff. She has several people on staff who could have supported her if she had know an effective way to delegate.

However, when she did delegate a task - she found herself micromanaging and hovering around them. It was like one of those helicopter parents making sure that every step was taken care of. The biggest reason she found herself micromanaging was this;

Nothing was written down.

There were no procedures, no policies, no job descriptions. There was nothing written down that the person she delegated to could follow - so that my client could be assured the proper steps were being taken. This is one of the key growth task of a small business - especially if you are a solopreneur - to create written policies and procedures so that you can hand them off to the people you would delegate to.

Now, there are two ways to do that but the key is this - get it out of your head. Everything that resides only in the head of the owner of the business is a problem. It cannot be delegated and nobody else can help you out and if it is not written down and you try to delegate you are going to hover and micromanage just like my client did.

So there are two ways to do this.

1) Do it Yourself:

Take those activities you would like to delegate and create the policy, create the procedure, a checklist or an algorithm yourself. That way, when you do hire someone in you could simply hand those documents off as part of the delegation process and know that they will get done at least as well as you are doing them right now.

2) Delegate it as Task #1:

When you hire that person in that you want to delegate to - make their first task be to write down these policies and procedures, Their first task is to create your Policy and Procedure Manual. What will happen is this; the first time you sit down with them to teach them the task you want them to take over - at the same time have them also write down the procedure in the form of a checklist or algorithm. This way you only have to teach them once and they both learn the task and create a written record.

Now, it is always important to be open to the fact that they may do it much better than you do and they may revise your procedure, but as long as the procedure remains written down it can be delegated again. Remember, if you are a solopreneur or a small business owner this first hire may turn out to be the head of a department a year or two from now - so giving you them the ability to delegate this task is important too.

Now, let me give you my one take home warning about policies and procedures.

It goes like this. You have to ensure that as you develop written Policies and Procedures that everyone remains Outcome Focused. The reason to be Outcome Focused is Outcome always overrides the Procedure.

Here is what I mean: If you create a policy or procedure manual you will find that a lot of people will actually key their actions off with the policy and forget about the outcome it was focused on. Remember that every policy and procedure is focused on creating a particular outcome - standardizing that outcome so it happens every time.

So if you have somebody who says there is a bad outcome in the business - there is a disgruntled customer or a client does not get a shipment on time - and the person that you have hired in says, "Well, I did all the steps in the procedure," and shrugs their shoulders.

That person has become procedure focused, not outcome focused. Remind them that this procedure was put in place to reach an outcome and if their procedure is not hitting the outcome, the outcome over rides the procedure. Give the customer what they want and need to make sure that you hit the outcome rather than just following the procedure and leaving the customer hanging.

This is always the danger when you create policies and procedures. It turns into something like "that is the way we do things around here". And anybody who has had a terrible customer service experience - and I know you have, I know I have - is because somebody was trying to follow a procedure and not trying to make you happy which is the outcome I am sure that company was looking for.

So let us recap.

If you have an activity you want to delegate write it down or have your first hire come in and write it down as one of their first tasks.

AND anytime you create a policy and procedure, make sure that you and the people you are delegating to recognize that it is focused on a outcome and remain outcome focused because the outcome is what is important to your business. That is it for now. Have a great rest today and a great week. Write down those policies and procedures. Get them all on paper and that will make your life a lot easier. Make it so that you do not have to micromanage and hover. Just remember, outcomes first. Here's to your success.

The Florida legislature ended its 2012 regular session the night of March 9 and did not pass a bill that would have authorized an annual Thoroughbred meet for Hialeah Park.

Under the proposal, Hialeah would have been able to hold a Thoroughbred meet each November provided that it was selected to be in a rotation of tracks to hold the Breeders? Cup World Championships. The track has not had Thoroughbred races since 2001 but has applied for some dates in 2013.

The legislature did not pass a bill that would have changed a Florida law?s definition of horse racing. The change would have prohibited the Florida Division of Pari-Mutuel Wagering from issuing new licenses under which holders could conduct pari-mutuel barrel racing meets.

The legislature also ended its session without passing a high-profile bill that would have authorized as many as three ?destination? resorts with hotels and full-scale casinos in the state. In addition, it did not pass a bill that would have allowed Greyhound tracks to stop holding live racing.

Going into the final day there was a consensus expectation, which proved correct, that the legislature would not pass any gaming-related bills. Early in the two-month session several House leaders said they would prevent the full House from considering any bills that could be viewed as expansion of gaming.

There are widespread expectations no gaming bills will be considered by the legislature during any special sessions this year, Steve Geller, a former president of the Florida Senate, said March 8. If anything related to gaming were added to a bill, ?it would kill a bill,? said Geller, a former president of the National Council of Legislators from Gaming States.

Geller is a partner in law firm Greenspoon Marder in Fort Lauderdale. He is a lobbyist for Mardi Gras Casino Florida, a Greyhound track/casino in Hallandale Beach. Geller expects that prospects for gaming bills will be better next year when the House has new leadership.

Apple addiction drives sales, hits retail margins

NEW YORK (Reuters) - For U.S. consumer electronics chains Best Buy and RadioShack , selling Apple products is at best a bittersweet experience.

Every new Apple gadget, such as the 4G iPad unveiled on Wednesday, helps bring more people into stores of the two chains.

At the same time, they are making less money with every Apple iPad, iPhone or iPod they sell than they would selling a comparable device.

"At a very basic level, Apple gives less retailer margin" because Apple creates the demand, said Andy Hargreaves, an analyst with Pacific Crest Securities.

Analysts have estimated that Best Buy buys the 16 GB iPhone 4S from Apple for about $600 then sells it for $300 to consumers who sign up for a new two-year wireless contract. Best Buy does not actually take a loss on the sale because wireless providers like AT&T give it a $400 commission for signing up the subscriber, but it makes only a $100 profit.

On a comparable Android device such as a Droid RAZR, Best Buy pays Motorola $300 for the phone, sells it for $200, and still gets the $400 commission, making $300 on that sale.

Best Buy's profit margin on the iPhone sale is about 17 percent compared with a profit margin of about 100?percent?on a comparable Android phone.

"Clearly, the profit margins on Apple products are thinner than other products, whether you are talking about an iPod versus another mp3 player, whether you are talking about an iPhone versus Android or BlackBerry handsets," said Anthony Chukumba, an analyst with BB&T Capital Markets.

He said that is also true of profit margins on an iPad versus an Android tablet.

Many iPads are wi-fi only, so retailers do not get paid by a wireless carrier on many iPad sales.

GOTTA HAVE 'EM

Regardless of how much retailers make on electronics sales, they must stock hot products.

"There is just not a whole heck of a lot that a Best Buy or a RadioShack can do about it," Chukumba said. "They have to carry Apple products from a sales perspective, a relevance perspective, and a customer traffic generation perspective."

The new iPad "will be a great upgrade product which will drive traffic and excitement at Best Buy, RadioShack, Staples Canada, Wal-Mart and Target," Janney Capital Markets analyst David Strasser wrote in a note on Thursday.

Chains like Best Buy and RadioShack have been struggling with narrowing margins, partly due to Apple products.

"When I have talked to Best Buy, they have told me the one exception is the Macs," Chukumba said, adding that Best Buy has better profit margins on Mac computers than on Windows-based personal computers.

Best Buy and Apple declined to comment for this story.

Unlike last year when only AT&T had the iPhone, all of RadioShack's major wireless partners, including Sprint and Verizon, are now pushing the iPhone. RadioShack Chief Executive Jim Gooch said last month that increased sales at RadioShack stores open at least a year but hurt margins.

DISCOURAGING FOR INVESTORS

Weak margins have driven away some investors. Anthony Abbate, a portfolio manager at Granite Value Capital, said taking a short-term hit to margins to increase sales is not a smart strategy for any company. "I can't really say an example of when it would be good to do that," Abbate said. "Overall, I think it's a really poor strategy because all you are doing basically is selling more units, but you are selling them at a lower price. So, at the end of the day, I don't think your profits are going to be any higher."

Abbate sold Best Buy's stock last fall citing poor business fundamentals.

In addition to the lower margin on many Apple products, Apple's clout in the industry is another factor working against retailers.

For example, if a non-Apple device did not sell well and had to be discounted, Best Buy could force some compensation from the manufacturer for the sales miss.?But, Chukumba said, "It wouldn't surprise me if Apple is not writing that check at all. Once again, it has to do with leverage. Apple has much more leverage over Best Buy than Samsung, LG, Sony, Motorola Mobility, HTC. There is just no comparison."

IN ACCESSORIES WE TRUST

Accessories and services can help boost margins for retailers selling Apple products.

"When somebody comes into Best Buy and buys an iPhone, make sure they don't just leave with an iPhone. Make sure they also buy a protective case, a bluetooth headset and maybe a warranty plan," Chukumba said.

Also, the high-end model of the new iPad can operate on a high-speed 4G Long-Term Evolution (LTE) network, presenting an opportunity for better margins if retailers sell more connections to people wanting faster service, Strasser said.

Best Buy should also encourage manufacturers to create products that can compete with Apple and carry higher margins. "Best Buy would love nothing more than there to be competitive Android tablets," said Chukumba.

EU's Ashton accepts Iranian offer of nuclear talks

BRUSSELS (Reuters) - European Union foreign policy chief Catherine Ashton wrote to Iran's chief nuclear negotiator Tuesday, accepting an offer to meet to discuss Tehran's nuclear program.

Ashton represents six powers - the United States, Russia, China, France, Britain and Germany - in dealings with Iran, and her offer of talks came after weeks of consultations with them.

It follows the expansion of sanctions by Europe and the United States to exert economic pressure and force Tehran to hold back on its nuclear program, which they fear aims to produce atomic weapons. Iran says it is enriching uranium for peaceful purposes only.

In a letter last month, Iran's chief nuclear negotiator, Saeed Jalili, proposed that negotiations with global powers should resume after more than a year's standstill, and said Tehran would have "new initiatives" to bring to the table.

"Today I have replied to Dr Jalili's letter of February 14," Ashton said in a statement. "I have offered to resume talks with Iran on the nuclear issue," she said.

Ashton's letter proposed an initial round of talks to focus on building confidence by developing concrete steps for the future.

"Our overall goal remains a comprehensive negotiated, long-term solution which restores international confidence in the exclusively peaceful nature of Iran's nuclear program, while respecting Iran's right to the peaceful use of nuclear energy," she said in her reply to Jalili.

Ashton said the time and venue for talks should now be decided but noted she wanted talks to resume as soon as possible.

"In practical terms," she wrote, "our deputies could get together in the near future in order to prepare for the first round of our resumed talks."

In another possible step toward greater cooperation, Iran said Tuesday it would let U.N. nuclear investigators visit a military complex where they had been refused access, to check intelligence suggesting Tehran has pursued explosives research relevant to nuclear weapons.

Western states are likely to tread cautiously, mindful of past accusations that Iran's willingness to talk has been a tool to buy time and not a path to agreement.

Officials say Iran faces an unprecedented strain on its economy due to the expansion of sanctions on its oil and financial sectors. In July, an EU embargo on Iranian crude is due to take full effect, drastically shrinking its export markets.

Netanyahu leaves US with assurances on Iran

Israeli leader Benjamin Netanyahu left Washington with assurances that the United States is prepared to take military action to prevent Iran from getting a nuclear weapon, just not yet.

Netanyahu, who met with President Barack Obama on Monday and Secretary of State Hillary Clinton on Tuesday, put the world on notice that his patience was wearing thin and, if necessary, he would launch unilateral strikes.

"As prime minister of Israel, I will never let my people live in the shadow of annihilation," he told 13,000 delegates in a keynote speech on Monday night at the American Israel Public Affairs Committee (AIPAC) conference.

"Unfortunately, Iran's nuclear program has continued to march forward. Israel has waited... for diplomacy to work, we've waited for sanctions to work. None of us can afford to wait much longer."

Sitting alongside the president at the White House before going into talks on Monday, Netanyahu told Obama that Israel must remain the "master of its fate," in a firm defense of its right to mount a unilateral strike.

Obama, who assured Netanyahu that he has Israel's "back," stressed that he sees a "window" for diplomacy with Iran, despite rampant speculation that Israel could soon mount a risky go-it-alone military operation.

While no one knows exactly what was said behind closed doors in the Oval Office, Obama publicly kept to a far more dovish line and appeared notably at odds with Netanyahu over just how immediate the Iranian threat is.

"This notion that somehow we have a choice to make in the next week or two weeks or month or two months is not borne out by the facts," he told a press conference Tuesday.

He spoke after world powers responded to Iran's new willingness to discuss the nuclear issue with an offer of talks, which Obama said would "quickly" show whether the Islamic republic was serious about avoiding war.

Obama, seeking a second term in November, argued that Iran was now feeling the "bite" of tightening sanctions though cautioned he did not expect a breakthrough in a first set of negotiations.

He also slammed Republican candidates for their hawkish statements demanding military action on Iran, after leading candidate Mitt Romney earlier said "thugs and tyrants" only understood American readiness to use power.

"This is not a game, and there's nothing casual about it," Obama said.

After meeting Clinton, Netanyahu held talks Tuesday with congressional leaders before flying home.

"We've had a very good visit in Washington, first in our discussion with the president in the Oval Office... and now culminating in this remarkable display of solidarity here in the Congress of the United States," he said.

"I go back to Israel feeling that we have great friends in Washington."

House Speaker John Boehner, a Republican, said: "The looming threat of a nuclear Iran cannot be ignored. Now is the time to stand together and we are here today to tell the prime minister that Congress intends to do so."

In Brussels, EU foreign policy chief Catherine Ashton, speaking on behalf of Britain, China, France, Germany, Russia and the United States, reiterated an offer to resume talks with Tehran.

The Obama administration says it does not believe Iran has taken a decision to develop a nuclear weapon, or that the time is right for military action, preferring to give biting new sanctions time to work.

But Israel, which sees a possible Iranian nuclear weapon as a threat to its very existence, claims Iran may be on the cusp of "breakout" capability -- when it could quickly build a nuclear weapon.

In his speech to AIPAC, Netanyahu sought to minimize the differences between himself and the US president.

(AP) ? Opposition leaders and Russian observers said Sunday they are seeing widespread violations in elections that are expected to return Vladimir Putin to the Kremlin.

Putin, who was president in 2000-2008, is expected to easily win the Sunday election against four challengers. But if credible evidence of vote manipulation emerges, it would bolster the determination of opposition forces to continue the unprecedented wave of protests that arose in December.

The independent elections watchdog agency Golos said it was receiving reports of so-called "carousel voting," in which busloads of voters are driven around to cast ballots multiple times.

"There have been many people voting more than once, driven around in buses in large numbers" in Moscow, said Golos head Lilia Shibanova, who said similar reports had been received from Novosibirsk, Russia's third-largest city, and the city of Barnaul in southern Siberia.

"We, of course, expected carousels, but not on this scale," Alexei Navalny, one of the opposition's most charismatic leaders, said on Twitter.

Allegations of widespread vote fraud in parliamentary elections set off the massive protests against Putin, who has remained Russia's paramount leader despite stepping down from president to prime minister four years ago due to term limits. They were the largest public show of anger in post-Soviet Russia and demonstrated growing frustration with corruption and political ossification under Putin, who as prime minister has remained Russia's dominant politician.

Golos' website recorded well more than 1,000 complaints of irregularities, including voter lists of questionable validity and nonfunctioning cameras in voting stations.

Web cameras were installed in Russia's more than 90,000 polling stations after the December election, a move initiated by Putin in response to complaints of ballot stuffing and fraudulent counts. Those elections saw his United Russia party retain its majority in parliament, though substantially reduced from its previous overwhelming control.

It was unclear Sunday to what extent the cameras would be effective in recording voting irregularities or questionable counts. The election observation mission of the Organization for Security and Cooperation in Europe noted skepticism in a report on election preparations.

"This is not an election ... it is an imitation," said Boris Nemtsov, another prominent opposition leader.

But despite the increased dismay, opinions polls have shown Putin positioned to easily defeat four other candidates and return to the presidency. Putin presided over a significant growth in Russia's prosperity and growing stability that contrasted with the disorder and anxiety of the 1990s, when Boris Yeltsin led Russia's emergence from the wreckage of the Soviet Union.

"Under Boris Nikolayevich, life was simply a nightmare, but, you know, now it's OK. Now it's good, I'm happy with the current situation," said 51-year-old Alexander Pshennikov, who cast his ballot for Putin at a Moscow polling station.

But other voters were tired of the heavy-handed ways of the one-time KGB spy. Natalya Yulskaya, 73, said she voted for billionaire Mikhail Prokhorov as a protest gesture against Putin.

"I know the KGB will be in power ... but I gave it a try," she said.

Putin has dismissed the protesters' complaints, portraying them as a coddled minority of urban elitists and as dupes of Western countries that he claims want to undermine Russia.

Putin's disdain for the protesters became more marked in the last week of campaigning, as he publicly suggested the opposition was willing to kill one of its own figures in order to stoke outrage against him. That claim came on the heels of state television reports that a plot by Chechen rebels to kill Putin right after the election had been foiled. Some of Putin's election rivals dismissed the report as a campaign trick to boost support for him.

Protests after the election appear certain.

"These elections are not free ... that's why we'll have protests tomorrow. We will not recognize the president as legitimate," said Mikhail Kasyanov, who was Putin's first prime minister before going into opposition.

The Interior Ministry called in 6,000 police reinforcements to the capital from other regions, the state news agency ITAR-Tass reported Friday. There were no immediate reports of trouble on Sunday, although police arrested three young women who stripped to the waist at the polling station where Putin cast his ballot; one of them had the word "thief" written on her bare back.

Tens of thousands of Russians had volunteered to be election observers, receiving training on how to recognize vote-rigging and record and report violations.

Putin has promised to appoint Medvedev prime minister if he wins the presidency in order to pursue his reform ideas, but many regard Medvedev as lacking the hard-edge political skills to be an effective reformer.

None of the other candidates have been able to marshal a serious challenge to Putin.

A mid-February survey by the independent Levada Center polling agency found Putin getting more than 60-percent support ? well above the 50 percent needed for a first-round win. The Communist Party candidate, Gennady Zyuganov, got support of about 15 percent, according to the survey, which claimed accuracy within 3.4 percentage points. The others ? nationalist firebrand Vladimir Zhirinovsky, Sergei Mironov of A Just Russia and Prokhorov ? were in single digits.

Locals are convinced that the light beam is a sign from the gods ? a warning about Dec. 21, 2012, the date that marks the end of the Mayan calendar cycle.

WASHINGTON (AP) ? A Republican congressman from Maryland is in a hairy situation over a proposal to give tax breaks to Americans with mustaches.

The American Mustache Institute claimed Tuesday that Rep. Roscoe Bartlett had lent his support to the 'Stache Act, which calls for a tax deduction of up to $250 a year for facial hair grooming.

But Bartlett's office said he never supported the measure. Staffers said Wednesday that they only forwarded a copy of the proposal to the House Ways & Means Committee, without the congressman's knowledge, after receiving a media inquiry about it. That led the institute to believe Bartlett, who has long had a mustache, supported the measure.

"For the record: Roscoe is pro-stache, but he does not believe Americans should pay for people's personal grooming decisions," Bartlett's chief of staff, Deborah Burrell, said in a statement.

So far, no other representatives have supported the mustache proposal.

At least one of Bartlett's Republican primary opponents is criticizing him over the facial hair flap. The longtime incumbent faces several challengers in the 6th District, which was redrawn to include more Democratic voters.

The American Mustache Institute, meanwhile, issued a statement faulting Burrell for what it called a "shameful reversal."

"We are highly disappointed by their reversal based on the fact that the congressman's opponents in the race are jumping on the bandwagon to criticize him," chairman Aaron Perlut said. "They obviously don't understand what it is to be a mustached American."

The institute plans to send Burrell an autographed photo of Burt Reynolds as a goodwill gesture, Perlut said.

State Delegate Kathy Azfali, who is challenging Bartlett in next month's primary, said the 'Stache Act situation was a sign that the 10-term incumbent had lost control of his staff and "is out of touch with voters."

Bartlett told WTOP-FM last fall that he grew his mustache in the 1950s as an affront to the clean-shaven.

"For someone who was kind of a nonconformist, it was kind of a symbol of rebellion," he said.

Bartlett supports the Movember campaign, which encourages men to grow mustaches in the month of November to raise money and awareness for men's health, specifically prostate cancer. The mustache institute has contributed to that campaign and other charities.

"I would encourage people who find out about the mustache institute's efforts to make tax-deductible charitable contributions toward the serious Movember philanthropy effort," said Lisa Wright, Bartlett's press secretary.

Wright said she forwarded the mustache institute's white paper on the 'Stache Act to the Ways & Means Committee because it's the committee's role to address media inquiries about tax policy.

"I looked at it, and I knew it was a joke," Wright said of the 'Stache Act.
